Home Health Aide (Hourly)

Assisting with bathing, hair care, personal hygiene, preparation for bed in evening, meals, light house-keeping, laundry; four hour minimum. $17.50/hour

Home Health Aide Visit

Same services as the home health aide hourly visit listed above, one hour visit. $25.00/visit

Companion / Homemaker (Hourly)

Provides companionship at home and to MD appointments, shopping, and errands, etc., as well as exercise, light housekeeping and meal preparation. $16.50/hour

Live-In Caregiver (Daily)

Provides in home care 24 hrs/day with full time caregiver. $185/day

Dietitians

Dietary counseling, meal planning and nutritional education. $95.00/visit

Skilled Nurse Visit

Provides wound care, rehab care, injections, post-op care, and set-up or administration of medications during single home care visit. $50.00/visit

Skilled Nurse (Hourly)

(Same as skilled nurse visit listed above. Services are offered by the hour with a 2 hour minimum). $35.00/hour

Nurse Case Management

Extended nursing coordination of client needs at home, such as coordinating multiple MD’s, ordering medications via mail, providing written client status reports to trust officers, families, etc. $75.00/hour

Physical / Speech / Occupational Therapy

Extended rehabilitation in your home. $115.00/hour
